Peri-implantitis and Treatment of the Peri-implant Lesion

In this lecture I shall tray to review the current knowledge on the incidence and pathogenesis of Periodontitis, as well as the most predictable therapeutic interventions to arrest the disease and in specific situations to also try to reconstruct the peri-implant lesion. Examples of the different surgical interventions and reconstructive therapeutic concepts will be provided.”

Forskningsprojekt med sponseret af DSOI: en redegørelse for projektet Faktorer som påvirker graden af periimplantitis

Der har inden for de seneste år været øget fokus på betydningen af forskellige risikofaktorer for udvikling af peri-implantitis. I forbindelse med gennemførelse af et forskningsår, som blev støttet af Dansk Selskab for Oral Implantologi og Dansk Tandlægeforening, blev sammenhængen mellem tidligere parodontal sygdom, rygning samt andre systemiske og lokale faktorers indflydelse på graden af periimplantitis belyst.
I alt 98 patienter blev henvist med henblik på diagnostik og behandling af periimplantitis og 34 patienter opfyldte inklusions- kriterierne. Almen og odontologisk anamnese blev optaget. Desuden blev der foretaget en grundig klinisk undersøgelse, inklusiv intraorale røntgenoptagelser af alle implantater.
Det blev konkluderet, at rygning og tidligere parodontal sygdom signifikant påvirkede graden af periimplantitis. Derudover kunne der ikke påvises yderligere forværring af periimplantitis ved samtidig tilstedeværelse af rygning og tidligere parodontalsygdom, sammenlignet med tilstedeværelse af én af disse faktorer.

“Nye” regler for brug af nye implantattyper hvordan påvirker disse regler mit arbejde?

Sundhedsstyrelsen udsendte den 13. januar 2014 en ny vejledning om lægers og tandlægers anvendelse af implantater. Vejledningen præciserer den omhu og samvittighedsfuldhed, som læger og tandlæger skal udvise ved anvendelse af implantater i patientbehandlingen. Den beskriver også regelsættene for, hvornår ibrugtagning af nye implantattyper og ny anvendelse af kendte implantattyper skal foregå som led i et forskningsprojekt, og hvornår det skal foregå med kvalitetskontrol.

In the nineties, there was a general belief that mechanical overloading was one of the main reasons for implant failure. Clinical and animal experimental studies on early and immediate implant loading provide information on the impact of mechanical loading on the establishment of osseointegration. Animal experimental studies also indicated the potential detrimental effect of excessive load on the sustainability of osseointegration, although controlled clinical trials are lacking. Research is challenged by the fact that clinical situations represents a blend of many influencing factors such as microbial and mechanical loading, surgical technique, prosthetics, patient-related factors such as bone quality and quantity, immune response, microcirculation, etc. This makes it difficult to evaluate the impact of a sole factor.

D vitamin betydning for knogleheling

Blandt voksne danskere har hovedparten et calciumindtag, som svarer til de nordiske anbefalinger på 800 mg/dag, mens kun meget få indtager D-vitamin i anbefalet mængde (10 mikrogram/ dag). D-vitamin og calcium modvirker sekundær hyperparatyroidisme, nedsætter knogleomsætningen og begrænser knogletabet. Tillige bedrer D-vitamin muskelfunktionen, hvilket mindsker risikoen for faldepisoder. Tilsammen fører disse effekter til en reduceret risiko for osteoporotiske frakturer Foruden at mindske risikoen for fraktur er der nyligt publiceret flere studier, som tyder på, at D-vitamin (evt. i kombination med calcium) har en generel positiv effekt på folkesundheden, herunder muligvis en forlænget levetid samt en reduceret risiko for cancer og en række kroniske sygdomme Lars Rejnmark vil fortælle om hvordan Dvitamin virker og hvordan det kan have betydning for vores patienter i forbindelse med knogleheling.
